Teeth whitening

Vital teeth whitening methods have precise indications as well as absolute or relative contraindications.

Indications for vital teeth whitening treatment:

  • genetic colouring (galena, grey-brown teeth);
  • age-related discolouration (ideal indication);
  • post-traumatic discolouration (attention to maintaining vitality);
  • tetracycline drug colouring (mild, medium forms);
  • fluroze (light, medium);
  • reducing the difference in colour between the remaining natural teeth and those with aesthetic unitary fixed prostheses (veneers or veneer crowns);
  • as a preliminary to prosthetic restorations by composite or ceramic veneer, to achieve a lighter colour shade in case of: severe tetracycline dyschromia (grade III, IV);
  • severe fluorosis (with structural changes of the enamel).

Absolute contraindications of vital teeth whitening treatment:

  • teeth with important structural alterations: fractures, fissures, dystrophic lesions;
  • teeth with severe dentine pain (hypersensitivity or hypersensitivity);
  • in patients with severe general (systemic) diseases;

Relative contraindications of tooth whitening treatment:

  • in women during pregnancy or breastfeeding;
  • in the teeth of children and young people under 14 years of age, in which the pulp chamber is bulky;
  • on teeth with bulky crown restorations, composite or amalgam.
  • We do not work under anaesthesia, so that we can detect pain in time;
  • The patient will be monitored and given some absolutely necessary information about the procedure: whitening will only discolour the teeth, not the pre-existing restorations. It may therefore become necessary to replace them after whitening;
  • Very dark shades of discolouration will only benefit from variable discolouration relief. After bleaching, discolouration may reappear;
  • Some stains do not respond to tooth whitening treatment. Teeth with amalgam restorations may appear darker in colour by contrast after whitening;
  • After the whitening procedure do not smoke, do not eat foods that can cause tooth discolouration (coffee, tea, red wine, etc).

Whichever tooth whitening method is chosen, before proceeding with the actual technique, some preoperative measures are necessary, which consist in:

  • performing a thorough clinical examination (anamnesis and rigorous objective examination), in order to highlight the aetiological factor, to assess the dental status and to evaluate the enamel quality of the teeth that will be subjected to whitening techniques;
  • removal of altered dentin and provisional restoration of carious lesions present on teeth to be whitened;
  • checking the tightness of the restorations present on the teeth to be whitened; periodontal assessment;
  • compulsory complementary examinations (X-rays, vitality tests);
  • assessment and notation in the consultation sheet of the colour shade of the teeth to be whitened and taking a photograph of their present condition.

  • A heat and light source can be used to release active oxygen;

Teeth whitening at home

A gentler alternative for more sensitive teeth is the technique of bleaching at home (by repeated applications of a low-concentration bleaching solution). This technique can also be used by braces wearers.
